I knowingly, willingly and freely choose to receive a sugaring/skincare treatment at Sweet Nothing Spa, and give my full consent to receive a service during the COVID-19 pandemic.
I understand the risks associated with receiving a sugaring/skincare treatment during the COVID-19 pandemic, which include the following:
• Coming in contact with someone else (another client, a service provider, another person in the building, etc.) that may/may not have signs/symptoms of COVID-19.
• Coming in contact with someone else (another client, a service provider, another person in the building, etc.) that may/may not have positive test results for COVID-19.
• Increased chances of contracting the disease simply by being in a public space where others (another client, a service provider, another person in the building, etc.) may frequent/have access to despite all cleaning/sanitizing/disinfecting procedures.
I attest to the following:
In the last 14 days I have not:
• Had a cough
• Had a fever greater than 100.4°F
• Had shortened of breath
• Been fatigued
• Had muscle aches/pains
• Had any new loss of taste or smell
• Traveled outside of the US
• Traveled outside of MA other than for essential work-related duties
• Had any contact with someone displaying any signs/symptoms of COVID-19
• Had any contact with anyone with positive test results for COVID-19
I agree that if ANY of the above statements apply to me, I will need to contact Sweet Nothing Spa at 929-235-0306 to reschedule my appointment.
Due to COVID-19, there are new requirements for ALL clients that must be followed in order to receive a sugaring/skincare service at Sweet Nothing Spa, and they are as follows:
• You must self-screen for signs and symptoms of COVID-19 before arriving to your appointment.
• You may not bring any guests with you to your appointment.
• You must call or send a text message once you arrive before coming to the building and wait for approval to enter.
• You must put on a face mask prior to entering the building. If you do not have a face mask, one will be provided to you for an additional charge of $5.
• You must wash your hands when you enter the building prior to starting your service.
• Payments for services must be made via a contactless system or cash (We cannot give change to you so please make sure you have the exact amount).
I voluntarily and knowingly accept all responsibility for receiving a sugaring/skincare service at Sweet Nothing Spa during the COVID-19 pandemic, have answered all questions truthfully and honestly, and release Sweet Nothing Spa of all liabilities and obligations and hold Sweet Nothing Spa harmless for any unintentional harm or exposure to COVID-19.
